Welcome to Zenith Health Ventures, LLC, doing business as HLTHI ("HLTHI"). By accessing or using our telehealth/telemedicine services, you agree to be bound by these Terms and Conditions.

Telehealth/Telemedicine Services

Telehealth/Telemedicine involves the use of electronic communications to enable health care providers at different locations to share individual patient medical information for the purpose of improving patient care. Providers may include primary care practitioners, specialists, nurse practitioners, registered nurses, medical assistants, and other healthcare providers who are part of the clinical care team.

Consent

By using HLTHI's telehealth/telemedicine services, you consent to the sharing of your protected health information with your clinical care team, family members, caregivers, legal representatives, or guardians for diagnosis, therapy, follow-up, and/or education. You also consent to the transmission of health information via Internet or telecommunication devices, which may include progress reports, bio-physiological data, and multimedia files.

Privacy and Confidentiality

The laws that protect the privacy and confidentiality of health and care information also apply to telehealth/telemedicine. Information obtained during telehealth/telemedicine sessions that identifies you will not be disclosed without your consent, except for treatment, education, billing, and healthcare operations purposes.

Security Risks

While HLTHI employs network and software security protocols to protect the confidentiality of patient identification and imaging data, and takes measures to safeguard the data and ensure its integrity against corruption, you acknowledge there is a risk of security breach. In the event of technical failures associated with the telehealth/telemedicine service, ZENITH HEALTH VENTURES LLC and all members of the care team are released from any loss of data or information.

Rights and Jurisdiction

This Agreement shall be governed by the laws of the State of Florida. Any legal action related to this Agreement shall be brought in the federal or state courts located in Hillsborough County, Florida, and both parties waive rights to a trial by jury. The prevailing party in any dispute will be entitled to reasonable attorneys' fees and costs.

Withdrawal of Consent

You have the right to withhold or withdraw consent to the use of telehealth/telemedicine services at any time without affecting future care or treatment.

This document represents the complete agreement between the patient and HLTHI regarding telehealth/telemedicine services.

Fees & Services

I understand that HLTHi is a virtual care service that connects patients with licensed medical professionals for online consultations. Before a visit takes place, my medical history will be reviewed, and I may be asked to complete additional forms if necessary. While most visits are completed within 24 hours of payment, delays may occur if more information is required by the provider.

HLTHi offers two optional membership plans: the Base Membership at $9.99/month and the Family Membership at $14.99/month. These plans do not include the cost of individual visits. If I am a member, the visit fee is $49.99. If I am not a member, the visit fee is $59.99.

The visit fee covers a consultation with a licensed medical doctor or a supervised medical assistant. It does not include additional services such as prescriptions, testing, or follow-up care beyond the visit itself.

If I request custom lab tests, the results will be shared with me along with an interpretation by a licensed practitioner. HLTHi does not provide treatment or follow-up care for any findings. I am responsible for seeking additional care from a primary care provider as needed.

I also understand that if the provider determines I am not medically appropriate for a consultation, I will not be entitled to a refund, as the visit fee includes the provider’s time and professional review of my case.
